When you request a quote or apply for insurance coverage, you are required by the various insurance companies we represent to disclose personal information about yourself to our agency. We appreciate the trust you place in us when you provide us with this information. We take our obligation to protect your information seriously. This notice provides you with an explanation of where and how we get information about you and to whom we disclose that information to. If the process of quoting, issuing or processing a policy, or handling claims, we may collect the following information about you from and share that information with but not limited to:

2. Phone numbers and email addresses
3. Date of Birth
4. Social Security Number
5. Driver’s License Number
6. Past insurance transactions including payment history and claims information
7. Consumer reporting agencies such as credit history
8. Government Agencies including county recorder’s office or motor vehicle department
